This is a care planning assessment folder comprising a user's manual, photocopy masters of checklist and interview forms and a PC disk from which copies of the forms can be made as an alternative to photocopying. It has been designed for use in care planning with a diverse group of elderly patients. The folder contains two versions. One is a checklist version, designed to be used by the key worker to assess behaviour within residential or day-care settings so as to inform the care planning. The other is the interview version. This is to be used where a client needs to be assessed from the perspective of the main caregiver, such as a family member. It is suitable for a home assessment or to use on admission to day or respite care in order to gain a more complete picture of the client's level of functioning and to give guidance in planning care. Both versions use a simple scoring method to assess self-care, memory and orientation, challenging behaviour, mood, sensory abilities and mobility.
