Simon's Nursing Assessment

An assessment procedure which determines the individual nursing care needs of older people with dementia. It assesses the following areas: memory/orientation; behaviour; mood disturbance; communication; sleep; washing/dressing; continence; nutrition; mobility; and skin care. Developed for use within continuing care settings for elderly mentally infirm people, it is intended to improve the standards of patient care and nursing practice within such settings, especially in the areas of individual needs assessment and care planning. The binder includes the following photocopiable resources: patient admission forms; care planning checklists; core care plans and multi-disciplinary patient review forms.
