This workbook provides a detailed guide to the development and use of the ten transitions that are critically important in assessing your organization's progress in moving towards the 21st century healthcare model.

Divided into four sections, the book progresses from establishing a basis for the ten transitions to providing a deeper analysis of each transition. Section I introduces the concepts that underlie the structure and methodology of the book.
