Concerted efforts are now being made to antedate the diagnosis of diabetic angiopathy in order to improve the chances for effective interventions in young people with diabetes and thus, hopefully, the prognosis of the disease. This publication summarizes current knowledge on the basic structural and functional events accompanying the development of diabetic angiopathy, the latest diagnostic procedures for their detection, and the therapeutic possibilities for children, adolescents and young adults in whom subclinical abnormalities are detected. Special attention is given to presently held pathophysiological concepts attempting to understand the development of neurovascular complications within a few years after the manifestation of diabetes. New information pertaining to abnormalities in collagen and protein glycan metabolism possibly leading to new methods or earlier diagnosis is also reviewed.
