Community Care Keyworker Manual

As community care develops, it will require new forms of case management and key care workers who are able to link services to service users in a way which is equitable, efficient and effective. This training manual is intended for use in the training of community care workers to help them focus their ability to assess need, empower service users, sustain networks of support and organize their own work properly, either within multidisciplinary teams or as solo workers. The manual includes checklists and practical exercises, charts, illustrations and diagrams as well as case study material.
