Despite repeated assurances that flying is safe, hardly a day goes by without a fatal aircraft crash. Many programmes have been published in an attempt to eliminate aircraft "accidents" but the problem still exists, apparently unabated. On a daily basis new pilots are injected into the aviation scene. Their safety consciousness is limited by the experience of their tutors. Intentional Safety is not taught.
