Transitions in Care

As a young person with type 1 diabetes transforms from pediatric patient into young adult, there is a transition in their health care as they move from pediatrician to adult care. At this critical juncture many of these patients are left without direction or guidance. This new book provides an effective solution to this growing dilemma. In part one, the book details diabetes care in the age of intensive therapy and examines the importance of transitioning the patient. It covers the issues of follow up and vulnerability to diabetes-related complications as well as the clinician's role in behavioral change. In part two, the authors discuss key developmental milestones and tasks facing young adolescents - from changing parent-child relationships to the development of an individual identity and the ability to make decisions as well as lifestyle changes. Lastly, part three presents a new paradigm of diabetes care which includes defining the patient-provider relationship, strategies for building commitment, changing self-care behavior, reducing fear of complications, preventing burnout, and more.
